CEDAR RAPIDS — A Cedar Rapids chiropractor pleaded not guilty today in U.S. District Court to multiple counts of mail fraud, aggravated identity theft and money laundering.

Douglas Dvorak, 45, was charged with 22 counts of mail fraud, 11 counts of aggravated identity theft and six counts of money laundering. The charges stem from a Medicaid scheme between late 2005 and early 2007.

A trial is set Dec. 1.

Dvorak allegedly obtained names and birth dates of adults and minor children in the Cedar Rapids area and fraudulently used the information without their knowledge or consent to file Medicaid claims, according to the indictment.

Dvorak solicited friends and other Medicaid patients to refer potential new patients to him, according to the indictment. He asked them to provide him with names and birth dates of individuals covered by Medicaid. He then contacted Iowa Medicaid Enterprise, administer of the program, and obtain their eligibility information.
